Pet Questionnaire

Thank you for choosing an NYC Pet Photography experience for your companion! One of the first steps in our journey together is for me to get to know you and your dog a little bit better. I want to know who they are, what makes them tick, and what their personality is like so that I can be sure to capture it during our photo session.

This form will take approximately 15 minutes and the questions are directed to your dog, so grab a treat for them and maybe a treat for yourself and let's get to know each other!
